This card is a simple tool for service dog owners. It will assist you when entering a business. The cards were designed to ease the burden of having to remember and relay federal guidelines which go along with owning a service dog. Now you just hand business owners one of these. On one side it says "I'm a Service Dog" and indicates that your dog is a Service Dog - an animal individually trained to perform tasks for people with disabilities. It has the toll free phone number to the U.S. Department of Justice should the business owner have any questions. On the flip side of the card is a summary of the Americans with Disabilities Act as it relates to Service Dogs. It outlines the questions businesses can ask and what they can't do.

  • "I'm a Service Dog"
  • Give it to businesses when you enter
  • Has the toll-free government number they can call if they have questions
  • Provides them with ADA information including what they can't ask (see below)

The back of the card says:

In accordance with the Americans with Disabilities Act of 1990

Businesses May ask:
   1) Is this a Service Dog?
   2) What tasks does the Service Dog perform?

Businesses May Not:
   1) Require special identification for the dog.
   2) Ask about the person's disability.
   3) Charge additional fees because of the dog.
   4) Refuse admittance, isolate, segregate, or treat this person less        favorably than other patrons.

A person with a disability cannot be asked to remove his service animal from the premises unless:

   1) the animal is out of control and the animal's owner does not take        effective action to control it.
   2) the animal poses a direct threat to the health or safety of others.

Any business that sells or prepares food must allow service animals in public areas even if state or local health codes prohibit animals on the premises.

Refusal to provide equal access to people with disabilities with service animals is a federal civil rights violation, provided by the American Disability Act of 1990. Violators of the ADA can be required to pay money damages and penalties.

When folded this card measures 3.5" x 2", the same size as a standard business card. You'll want to keep these tucked away in your service dog's vest.
